Heart at Home
Heart at Home is a specialized heart health program for patients living with chronic heart problems including heart failure, hypertension, coronary artery disease, stroke, and atrial fibrillation. With the Heart at Home program, we work closely with physicians – emphasizing education, self-assessment, and in-home treatments in order to relieve the symptoms of heart failure and prevent re-hospitalizations.
Our mission is to optimize your health by developing a personalized treatment plan that can be carried out in your own home, assisted living or other independent living community. We show you how to monitor your symptoms and follow a low sodium diet and exercise routine.
