Meet the Hospice Care Team

Hospice care is a coordinated effort between multiple professionals, each offering a unique kind of support. In addition to developing a comprehensive plan with the patient and caregiver, the Ascend Hospice team also works closely with the patient’s primary care doctor.

Medical Director
Oversees the entire care plan and collaborates with the care team. At Ascend Health, many of our medical directors are board-certified in hospice and palliative medicine.
Hospice Nurse
Manages medications and symptoms, provides skilled care, educates families, assesses the patient’s condition, and arranges for special medical services.
Home Health Aides
Assist with personal care like bathing, dressing, and grooming. They also offer comfort and companionship.
Social Workers
Provide emotional support, counseling, and help connect families with community resources and practical guidance.
Chaplains
Offer non-denominational spiritual and emotional support, respecting each family’s unique beliefs and values.
Volunteers
Offer companionship, run errands, and provide holistic services such as aromatherapy, Reiki, massage, and even pet therapy.

How Long is Hospice Care?

There’s no exact limit to how long hospice care lasts — it depends on the patient’s needs and overall condition. While initial eligibility is based on a physician’s prognosis of six months or less, patients can continue receiving hospice care beyond that timeframe if they still meet the criteria, with regular recertification from their doctor. In some cases, patients stabilize or improve and may be discharged from hospice care. If their condition changes later, they can re-enroll when appropriate.
